Instructions for Booking an Allergy/Immunology Appointment Online
START: You will receive a link to register by email and/or text. Click/tap that link.
Step 1: You will see the “Check-In” page in your browser. See example screenshot below.
a) Type in THE PATIENT’S First Name, Last Name, OHIP number, and Birthdate
b) Click the [Check-In] Button.
Step 2. You will see the “Select Provider” page in your browser. See example screenshot below.
a) Select ‘Dr. Aujnarain’
b) click the [Check-In button].
Step 3. You will see the “Provide Details” page in your browser. See example screenshot below. Note that this is a longer form. Please
a) fill out this form as best you can
b) click the [next] button at the end of the form.
Step 4: You will see the “Choose Time and Date” page in your browser. See example screenshot below.
a) Navigate and click/tap on the date of the appointment
b) choose the time of the appointment that you wish to schedule.
c) enter your email address so that we can send you a confirmation.
d) Tap/Click the [Book Appointment] button
Step 5: You will see the “Booking Confirmation” page in your browser. See example screenshot below.
a) Take note of your confirmed appointment date and time.
b) Note that this is a virtual visit so please do not come to clinic for this appointment.
c) Read and acknowledge the booking instructions by clicking on the checkboxes to the left.
d) click the [Book Appointment] button to complete the booking process.
